INTRODUCING CASE & COLDWELL'S
Health Provider Profiles Minicourse
Hi! I'm Marion
I'm a former care partner, caregiver and eldercare advocate. I was also a project management team (PMT for short) support specialist who enjoyed creating digital tools to organize data that helped managers make key decisions. These two distinct paths crossed when I needed to help make key care-related decisions for both parents when a sudden health change meant they needed 24/7 care.Â
I continue to hear stories of burnout that too often accompany care-anything. While my experience was unique, I've experienced the benefits of sharing accurate and timely information—like in the PMT—but within care systems. For best outcomes all around, I encourage current and future eldercare advocates to be proactive and do all they can to have as many shared basics in place as possible.
Gather basic information you need for your advocate journey and reduce stress as you build your sense of agency!

"TALKING WITH PARENTS WAS UNCOMFORTABLE AND OVERWHELMING AT FIRST, BUT THIS INFORMATION GUIDED US THROUGH THE RIGHT QUESTIONS.”
~ Lorrie S., Daughter and Future Care Partner
What's different?
Health provider profiles are most useful when shared and kept updated. At medical clinics or the ER, quick access is invaluable for anyone in an advocate role.
Assisted living, residential care homes, and the like typically offer blank pages for you to “fill in,” but sharing it amongst care partners can fall through the cracks.
Updating, copying, and distributing paper forms in a timely fashion is tedious... and may not happen when time is scarce. This can lead to decision-making based on inaccurate assumptions.
Our template lets you get ahead of assumptions, allowing you to digitally capture and safely share your organizing efforts.Â
Plus, you'll have the option to print whenever that makes sense for you. Paper... it's still helpful!
