"Reviewing information and planning together ahead of time brought our family peace and left me feeling better prepared for when that time comes to help out."

~ Gabe, Son & "better prepared" Future Care Partner

Does this sound like you?

  • Uncertain who your parent’s doctor is or how to quickly reach them in an emergency.
  • Resist talking with loved ones about health anything because it feels too awkward.
  • Unclear if parents keep their healthcare information organized or if you could access it if needed.
  • Beginning to worry about potential outcomes for both you and a loved one due to their health changes.

Imagine how it would feel to…

  • Have quick access to loved one's key health-provider information in an emergency.
  • Confidently advocate for loved ones when they are unable to advocate for themselves.
  • Replace worry with mindful action based on verified data, not assumptions.
  • Experience an increased sense of agency on your advocate journey.

Our template helps you build an easy-to-access and reliable resource - no matter where you're starting from - so you can advocate with knowledgenot assumptions.

Hi! I'm Marion,

a former care partner and eldercare advocate. I also worked on project management teams (PMT) where I created digital tools that organized data in support of management decisions. These paths crossed when I needed to help with care-related decisions for both parents after a sudden change in health meant 24/7 care.

I often hear stories of burnout related to care-anything. With loved ones, it's easy to extend yourself beyond a healthy capacity—it happens fast.

A solid step towards resiliency in the face of caregiving stress is removing guesswork wherever you can. Like in the PMT, there are practical benefits from having data organized and as many resources in place as possible before important decisions.

What's different?

Health provider profiles are most useful when they are easy to access and kept updated. In an emergency, at medical clinics or the ER, quick access is invaluable for someone in an advocate role. Our minicourse and template helps you prepare for those times.

Updating and printing paper forms in a timely fashion gets tedious... and may not happen when time is scarce. This can lead to decision-making based on inaccurate assumptions.

Our template lets you get ahead of assumptions, by providing ways to digitally capture and access your organizing efforts.

Plus, you'll have the option to print whenever that makes sense for your unique situation.

What's included:

  • Digital Health Provider Profiles Template

  • Printable PDF version of template

  • Minicourse with four lessons:

    • Clarify your advocate goals
    • Conversation strategies
    • What to know
    • Keep data updated
